Family Habilitation


Services are designed to assist parents or other caregivers to maintain or reunify their family unit. These services are provided primarily in the client's home and are protective services to assist in maintaining the family and strengthening the ability of parents or caregivers to provide adequate childcare and to improve parenting skills. Transportation can be provided to appointments to assist the parents or caregivers in maintaining or reunifying their family unit.

Family Services

Visitation


Services are to provide supervised visitation and transportation between children who have been placed out of the home and their parents and/or siblings. Services are designed to enhance the visitation process through weekly planned visits between parent and child, thereby maintaining the parent/child bond and promoting a timely plan for the child to return to the home of the parent whenever the permanency goal is to return home. The primary work of the visitation staff is to endure that regularly scheduled visitation with parents and family members occurs.

Youth Housing


Services are to provide assistance in locating and securing permanent, affordable housing. Services include:   Consumer education, Housing location, Securing affordable housing, Basic life skills information, Counseling on compliance with rental lease requirements, Job preparation, Education and career advancement counseling, Participation of youth in the assessment and implementation of actions to address their needs. These services are available for a DCFS ward or former ward.

Norman Housing


Services are to assist the client in locating and securing permanent, affordable housing. Services include: Consumer Education, Housing Location, Securing Affordable Housing, Assist the client in applying for Income Assistance, Linking the client to community resources, and follow-up with the client until he and/or she is stabilized in permanent, affordable housing. These services are to open family cases and referred by DCFS and has been certified as a member of the Norman Class.

Project Safe


Services are providing intervention, diagnosis, and treatment as well as providing concurrent interventions aimed at enhancing parental and family functioning. Services provided include outreach, parenting and related services aimed at enhancing parental functioning. (OASA) funds the substance abuse assessment and treatment portion of Project SAFE.

Required opening paperwork: Resource and referral, Release of Information, CFS 440-5 Adult Substance Abuse Screen, CFS 440-6 AODA Referral form and a current Consent for Disclosure-Substance abuse Assessment and or Treatment-CFS 440-7.


Prevention Services


Evidence based programs provided in schools. Community based programs implemented through coalition building.
