Community Support Services
Bridgeway’s Adult Coordinated Care Services are designed to assist persons to live effectively within the community, thereby reducing the disruption caused by the illness, repeated psychiatric admissions to mental health facilities, the extensive use of crisis services, incarceration, preventable admission to long-term, skilled nursing facilities or repeated use of emergency rooms. Community based services, such as Adult Coordinated Care Services, provide goal-oriented, individualized and accessible supports to improve self-sufficiency so that persons with psychiatric impairments can achieve recovery outcomes, personal growth and decreased need for costly institutional care.
